During the COVID-19 pandemic, our next Community Partner spotlight provided consistent and quality services. Transition House Emergency Shelter provides safe and supportive shelter for Northumberland County’s most compromised citizens. As the only emergency shelter that serves all citizens of Northumberland County, Transition House is committed to offering a valuable and often life-saving service for those with multiple challenges, particular vulnerabilities and needs related to homelessness and housing insecurity.
Transition House utilizes a Housing First philosophy, providing not only emergency shelter, but assistance in rapid rehousing by mobilizing a variety of community partners and supports. With an integrated and individualized approach, residents at Transition House take part in developing a personal action plan and staff work with them to access the appropriate community resources and services that are available in the community. Transition House has strong community partnerships and is an access point to a boarder system of supports and services.
At Transition House, they recognize that secure and lasting housing is fundamental to human dignity and is a basic human rights issue. They strive to create a supportive atmosphere where staff and residents work together to promote a safe, secure environment that provides stability and structure to the residents’ lives. Transition House provides a variety of services to their residents including goal setting and action planning, assistance finding permanent housing, financial management assistance, weekly house meetings, to name a few.
